Release and Waiver of Liability
I, the undersigned, desire to volunteer with Champlain Valley Office of Economic Opportunity (CVOEO) and engage in activities related to providing volunteer services. I acknowledge and agree to the following:
• Waiver and Release: I release and forever discharge and hold harmless CVOEO, its affiliated entities, directors, officers, employees, agents, and successors from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from my volunteer work. I understand and acknowledge that this Release discharges CVOEO from any liability or claim that I may have with respect to bodily injury, personal injury, illness, death, or property damage that may result from my activities as a volunteer, including those arising out of negligence.
• Insurance: I understand that CVOEO does not assume any responsibility for or obligation to provide me with financial or other assistance, including but not limited to medical, health, workers compensation, or disability benefits or insurance in the event of my injury, illness, death, or damage to my property that may result from my activities as a volunteer. I am responsible for my own insurance coverage. If my volunteer duties involve driving, I represent and warrant that I possess adequate automobile insurance coverage.
• Medical Treatment: I hereby release and forever discharge CVOEO from any claim whatsoever which arises or may hereafter arise on account of any first aid, treatment, or medical services rendered in connection with an emergency during my time as a volunteer.
• Assumption of the Risk: I understand that the activities I participate in as a volunteer may include work that is hazardous to me. I expressly and specifically assume all risks of injury or harm in connection with my volunteer activities.
• No Compensation: I understand that I am not an employee of CVOEO and will not receive compensation for my services. Reimbursement for pre-approved expenses (e.g. Mileage).
• Volunteer Insurance Notice I understand that CVOEO may maintain liability insurance that may apply to certain volunteer activities, but that this insurance is not health, accident, disability, or workers’ compensation coverage for volunteers unless the Organization specifically states otherwise in writing. I further understand that I am responsible for my own health, accident, and medical insurance coverage, and that I should not assume that CVOEO’s insurance will pay for my injuries or losses.
By signing below, I acknowledge that I have read, understood, and voluntarily agree to the terms and conditions of this Release and Waiver of Liability.
Thank you for volunteering! We appreciate your support of Feeding Champlain Valley, CVOEO and those that we serve!
